Parent must be present
State issued identification for the minor
(Learner’s permit, state issued ID, passport, or birth certificate are acceptable. Student IDs will not be accepted as an appropriate form of ID)

Notarized consent form
(We DO NOT have a notary on site. These documents need to be notarized prior to coming in for your appointment. Notaries can be completed at your local bank or at Amscot)
